Account Setup

To obtain an account: You must request (515-281-5247) or download the Application for Charge Account. Complete the application and fax or send it to the address on the form.

To obtain a PIN: If you already have an account then request (515-281-5247) or download the Electronic Services Application & Agreement. Complete the application and fax or send it to the address on the form. If you do not have an account you may fax or send both the Application for Charge Account and the Electronic Services Application & Agreement together.
